Community Health Promotion

Our Community Health Promotion program is powered by a dedicated team of Community Health Workers (CHWs) who serve as trusted connectors between individuals and essential services. Embedded in the communities we serve, CHWs help residents navigate health and social systems – whether it’s accessing food assistance, scheduling medical appointments, attending community health events or addressing other social determinants of health. They are at the frontline of improving health outcomes and strengthening community well-being.

Geriatric Workforce Enhancement Program (GWEP)

We have partnered with AAMU Department of Social Work assist in creating Age Friendly communities across north Alabama. Established in 2024, the Alabama A&M University Geriatric Workforce Enhancement Program (GWEP) is housed within the Department of Social Work on the Alabama A&M University campus.

The program aims to tackle Alabama’s significant health challenges and the shortage of healthcare professionals by integrating geriatrics into primary care systems and expanding the availability of skilled geriatric providers. Its efforts focus on improving training for healthcare providers, patients, caregivers, and other professionals, ultimately enhancing health outcomes.

We are engaged in two aspects of this broad reaching program.

Age Friendly Community Education

In coordination with our Community Health Workers, we will provide education to caregivers, healthcare workers and member of our community that work with the geriatric population including:

  • At-home Caregivers
  • Churches
  • Community groups
  • Senior Centers
  • Nursing, Healthcare facilities
  • Other organizations
